Using our new forms, you can request spare parts from us as well as initiate orders for service technician appointments or telephone support. The information you provide us with using these forms enables us to prepare an offer for spare parts more quickly and more efficiently, and send you confirmations for service appointments.

How do I use the forms?

Please download the form you require. If you have a spare part request, you need the “Spare parts RFQ” form, and if you want to make an appointment for a service technician or to receive telephone support, you require the “Service request” form. Simply click on the links above to open the form.

Please fill out the required fields on the form, which are marked with an asterisk (*). You can complete the form either by hand or on your computer.

Once you have entered all the required information, sign the form and send it via email to srvcpl-kstrd or via fax to +49 (2982) 92 11-105.

As soon as we have received the completed and signed form, we will process it and send you a quotation for spare parts or confirmation of a technician appointment or telephone support.
